Everybody knows what it's like to feel anxious the butterflies in your stomach before a first date, the tension you feel when your boss is angry, and the way your heart pounds if you're in danger. Anxiety rouses you to action. It gears you up to face a threatening situation. It makes you study harder for that exam, and keeps you on your toes when you're making a speech. In general, it helps you cope. But if you have an anxiety disorder, this normally helpful emotion can do just the opposite it can keep you from coping and can disrupt your daily life. There are several types of anxiety disorders, each with their own distinct features. An anxiety disorder may make you feel anxious most of the time, without any apparent reason. Or the anxious feelings may be so uncomfortable that to avoid them you may stop some everyday activities. Or you may have occasional bouts of anxiety so intense they terrify and immobilize you.

They can't predict when an attack will occur, and many develop intense anxiety between episodes, worrying when and where the next one will strike. In between times there is a persistent, lingering worry that another attack could come any minute. When a panic attack strikes, most likely your heart pounds and you may feel sweaty, weak, faint, or dizzy. Your hands may tingle or feel numb, and you might feel flushed or chilled. You may have chest pain or smothering sensations, a sense of unreality, or fear of impending doom or loss of control. You may genuinely believe you're having a heart attack or stroke, losing your mind, or on the verge of death. Attacks can occur any time, even during nondream sleep. While most attacks average a couple of minutes, occasionally they can go on for up to 10 minutes. In rare cases, they may last an hour or more. Panic disorder strikes between 3 and 6 million Americans, and is twice as common in women as in men. It can appear at any agein children or in the elderlybut most often it begins in young adults. Not everyone who experiences panic attacks will develop panic disorder for example, many people have one attack but never have another. For those who do have panic disorder, though, it's important to seek treatment. Untreated, the disorder can become very disabling.

    Study Description
    . Panic disorder is an illness in which a person experiences sudden, sometimes unexpected rushes of intense fear or discomfort accompanied by a number of distressing physical sensations. These episodes, called panic attacks, often are so frightening or uncomfortable that sufferers avoid places or situations where they feel either more vulnerable to attacks or more cut off from help or safety. This kind of avoidance is called agoraphobia. There are several effective treatments for panic disorder. A commonly recommended one is a form of psychotherapy called cognitive behavior therapy (CBT), which focuses on reducing the person's fear of panic symptoms and assisting the person to resume avoided activities. Another common form of treatment is medication treatment with an "antidepressant" medication. In a previous study conducted simultaneously at CARD and three other universities, we compared the effectiveness of CBT, the antidepressant drug imipramine (at that time, the best studied antidepressant drug for panic disorder), and the combination of CBT and imipramine in more than 300 people with panic disorder. The study found that improvement was as great with either CBT or imipramine alone as with their combination. Given the added cost and effort of combined treatments, it therefore seems appropriate to begin with a single form of therapy. The question then is what to do next-what to do with patients who respond well to the initial treatment as well as those who do not. This study addresses those questions. The study is supported by a grant from the National Institute of Mental Health and is being conducted at the same four universities where the earlier study was done.

    What is it?
    A panic attack is a sudden onset of intense apprehension, fear, or terror that is often associated with feelings of impending doom. It also involves the sudden onset of intense physical sensations or symptoms. A panic attack is different from other types of anxiety in that it involves an intense rush of fear or physical sensations that reach a peak very quickly, usually within ten minutes. During a panic attack, people may feel that things around them are not real, that they’re going to die or they they’re going crazy. Physical symptoms associated with a panic attack include shortness of breath, dizziness, trembling, sweating, choking, and heart palpitations.
    Panic attacks often occur in feared situations such as giving a speech, flying, seeing a snake, or other situations that usually make a person fearful. For some people, however, panic attacks occur spontaneously or “out of the blue”, leading a person to think that s/he is having a heart attack or a terrible undiagnosed illness.
    Panic Disorder , which affects 2% to 3% of the adult population, is diagnosed when spontaneous panic attacks frequently occur and the person experiencing them spends a significant amount of time worrying about the next panic attack. People with Panic Disorder usually limit their social and vocational activities because of fear of having another panic attack.

The following brief narrative describes what a person with panic disorder might experience: The first time it happened, I was going to the mall to do my Christmas shopping. The mall was very busy and I was going up a crowded escalator. I suddenly felt trapped. My heart was beating beat so fast I thought my chest would burst. I felt faint and started sweating and was very scared. I thought I was going to die. I got off the escalator and made it over to a nearby bench. My legs felt wobbly. I was afraid I wouldn't make it to the bench. After a few minutes, a security guard came over to ask me if I was all right. I looked up at him and told him that I felt very sick. As I talked to him, the feeling started to go away. I was able to drive myself home. The feelings expressed by the person who had a panic attack in the mall are severe symptoms of anxiety:
