Origin
The Expanded Food and Nutrition Program (EFNEP) began as a U.S. Department of Agriculture (USDA) Extension pilot project in the 1960s during a time of increased awareness of hunger and malnutrition in the United States. In 1969, as a result of the pilot studies, effective approaches for contracting, teaching and maintaining programs for low-income audiences were identified and the Cooperative Extension nework was given responsibility for the program. In 1970, EFNEP implementation begain in New York City, with three program sites in the counties of Kings (Brooklyn), Bronx, and New York City (Manhatten). Mission
To enhance the ability of adults and youth residing in limited resource communities to make informed decisions related to food and nutrition, resource management, parenting, and health care that contributes to improved health, self-esteem, and self-sufficiency. Objectives

The Farmers' Market Nutrition Program began as a U.S. Department of Agriculture initiative in 1988 with a pilot project in New York City. What began as 10 markets throughout New York City has increased to 41. Cornell University Cooperative Extension - NYC Programs began nutiriton education in four of the markets during that first year. The number of markets receiving nutrition education services has grown to 11. Mission
Objectives

  • Provide food demonstrations using fruit and vegetables available at farmers' markets Offer opportunities to sample food and vegetable items prepared at food demonstrations Provide education at WIC clinics about use of Farmers' Market Nutrition Program coupons Provide education about unfamiliar fruit and vegetable items Increase number of farmers' markets that receive nutrition education services Train nutrition and dietetic interns and volunteers to deliver nutrition education about fruits and vegetables at farmers' markets.
Target Audience

Senior Nutrition Programs
HUNGER AND THE ELDERLY
Individuals aged 60 and older are one of the most vulnerable populations affected by hunger. In New York State, it is estimated that one out of every four elderly New Yorkers living at home is considered nutritionally at risk. This translates to a staggering 728,000 New Yorkers aged 60 or older at risk of malnutrition. Data on a national level equally demonstrate astonishingly high levels of hunger among the elderly. In a GAO report (August 2000) almost 2 million households aged 60 or older indicated that they either did not have enough of the right types of food needed to maintain their health or simply did not have enough to eat. Good health is closely linked to diet. When an elderly person reduces food intake due to health problems or financial restrictions, they in fact risk their ability to maintain independent, healthy lifestyles. Nutrition assistance programs are available under the Older Americans Act to reduce hunger for the elderly. More can be done to promote the nutrition assistance services available. The Food Stamp Program through the USDA is the primary, federal nutrition assistance program for low-income elderly New Yorkers that is always available for those who are eligible. Unfortunately, it is estimated only 17% of eligible seniors in upstate New York are enrolled in the Food Stamp Program.

Women, Infants, and Children (WIC) 2002 - 2003: A fact sheet for New York State Children's healthy development depends in large part upon the first few years of life. Malnutrition during the fetal stage or early childhood can cause serious damage to a child's physical and mental well-being. Low birth weight increases a child's risk of mental retardation, physical handicaps, illnesses, and infant mortality. The Women, Infants, and Children (WIC) Program is designed to improve nutrition among pregnant and post-partum women and young children. In New York State, the program is run through the New York State Department of Health. What services are provided? What is WIC's track record? The U.S. Department of Agriculture reports that WIC has been proven to:
  • Reduce the incidence of very low birth weight babies by 44%. Return an estimated of $3.50 in savings in federal, state, local and private health care costs for every $1 spent on WIC. The Office of School Health Programs provides staff development, consultation and technical assistance in comprehensive health education for elementary and middle school personnel that focuses on how to address health risk behaviors with students. Staff development provides teachers and other school personnel with an understanding of comprehensive health education, including growth and development, substance abuse prevention, safety, nutrition, disease prevention, emotional health, and personal health and hygiene. Implementing Cardiovascular Disease Guidelines for Physicians and Patients to Improve Health Outcomes
Public Health Problem
Cardiovascular disease (CVD), mainly heart disease and stroke, is the leading cause of death in Arkansas. The state ranks fifth in the country in deaths from heart disease and second in deaths from stroke. The increased burden of disease in Arkansas compared with the rest of the United States may be partially explained by the higher rates of cardiovascular risk factors among state residents. Behavioral Risk Factor Surveillance System (BRFSS) 3099 data indicate that more people in Arkansas than those in the general U.S. population have high blood pressure (28% vs. 24%), smoke cigarettes (25% vs. 22%), and are completely physically inactive (28% vs. 27%). Evidence That Prevention Works
Compelling evidence from recent clinical trials supports the merits of aggressive risk reduction therapies for patients with CVD. The American Heart Association and the American College of Cardiology urge all health care settings where CVD patients are treated to develop specific protocols and procedures reminding health care providers to implement the guidelines and assess the success of appropriate treatments.

For effective nutrition programming, planning and policymaking, state and community leaders need timely, objective information on the current and changing nutrition condition of the populations they serve. Providing such information is the goal of nutrition surveillance. The NYS Nutrition Surveillance Program is one of the first state-level surveillance efforts in the U.S., and is unique in its university-state agency partnership. It is nationally recognized for innovative efforts at both the state and local levels. The New York State Nutrition Surveillance Program at Cornell began in 1984 as a collaborative effort of the Division of Nutritional Sciences, Cornell University (DNS) and the Bureau of Nutrition, New York State Department of Health (DOH). In recent years, the Cornell group has worked to document nutrition issues and support effective programming, planning and policymaking in New York State. The DOH both funds and collaborates on the activities conducted by Cornell, and conducts other surveillance activities throughout New York State.

Mayville, New York 14757 The Chautauqua County Office for the Aging, (CCOFA), a governmental Area Agency on Aging has, since 1973, maximized senior nutrition services with creative cooperative efforts, quality control and client-based programs. In 1996, the Senior Nutrition Program served 289,987 meals to 2,298 participants through direct congregate program operation and subcontracted home-delivered meals. Utilizing the 1997 budget of $1,276,822, we expect to continue our five-year policy of "no waiting lists/no one unserved" despite lessening funding, by actively continuing to work with meal suppliers, Jamestown Public Schools and Brooks Memorial Hospital and Meals on Wheels of Jamestown Area, Sinclairville 76ers and Dunkirk Fredonia Meals on Wheels. Participants in the 18 Congregate Nutrition Program "Dining Centers" may select a hot or cold meal (both supplying 1/3 of the RDA) with a 24 hour in advance reservation. Home-Delivered Meal participants receive a hot main meal and many also receive a cold meal delivery at the same time. Weekend shelf stable and frozen meals are available for the HDM participant as well as a four pack "Blizzard Box." With the establishment of the first congregate nutrition program in New York State in 1973, the Senior Nutrition Program set a precedent of client based services, that continues to the present. The first meal in New York State was served April 1, 1974. Demographically and philosophically different, the two primary population centers of Chautauqua County, Jamestown and Dunkirk-Fredonia have permitted, and at times, necessitated customized nutrition services. With only two population centers, the 1069 square miles of the county also require planning for very rural areas.
