Gene Polymorphisms and Altered Risk of Adult Acute Lymphocytic Leukemia Martyn T. Smith

Background : Although the clinical and pathological aspects of leukemia are well known, little is understood about the genes that influence susceptibility to this complex disease. Certain gene polymorphisms have been shown to alter the risk of development of leukemia and these variations can interact with diet, other environmental exposures, and individual immune function to be major determinants of susceptibility. This researcher has previously reported polymorphisms in a folate metabolizing gene with a decreased risk of acute adult lymphocytic leukemia (ALL). The research hypothesis is that the protective effect is due to an increase in the flux of folate compounds available for DNA synthesis and subsequent reductions of uracil in the DNA. Accumulation of uracil in DNA and its subsequent removal during excision repair processes can result in DNA double strand breaks which are necessary for chromosomal translocations and deletions. The Atkins diet is a high-protein, high-fat, and very low-carbohydrate regimen. It emphasizes meat, cheese, and eggs, while discouraging foods such as bread, pasta, fruit, and sugar. It is a form of ketogenic diet. Purpose The primary benefit of the diet is rapid and substantial weight loss. By restricting carbohydrate intake, the body will burn more fat stored in the body. Since there are no limits on the amount of calories or quantities of foods allowed on the diet, there is little hunger between meals. According to Atkins, the diet can alleviate symptoms of conditions such as fatigue , irritability, headaches, depression, and some types of joint and muscle pain Description The regimen is a low-carbohydrate, or ketogenic diet, characterized by initial rapid weight loss, usually due to water loss. Drastically reducing the amount of carbohydrate intake causes liver and muscle glycogen loss, which has a strong but temporary diuretic effect. Long-term weight loss occurs because with a low amount of carbohydrate intake, the body burns stored fat for energy. The four-step diet starts with a two-week induction program designed to rebalance an individual's metabolism. Unlimited amounts of fat and protein are allowed but carbohydrate intake is restricted to 15-20 grams per day. Foods allowed include butter, oil, meat, poultry, fish, eggs, cheese, and cream. The daily amount of carbohydrates allowed equals about three cups of salad vegetables, such as lettuce, cucumbers, and celery.

The theory that a high-fiber diet wards off colon cancer, the second-leading cancer killer in the U.S., had been around since the 1970s. It all started when scientists noticed Westerners got more colon cancer than poor people in rural Africa. While the differences are numerous, they believed the Africans' higher consumption of fiber was a significant factor. Fiber makes the stool bulkier and thus probably more likely to dilute carcinogens hanging around in the intestine. Also, the fiber would move any carcinogens along more quickly. The theory made sense. It seemed convincing enough for health agencies to recommend a high-fiber diet to prevent colon cancer, even though the research evidence was weak or unclear. The concept of fiber preventing colon cancer began to erode when results from several studies started confounding this conventional wisdom. What is leukemia Leukemia is cancer of the blood cells. (Read about " Cancer: What It Is ") Blood cells are formed in the bone marrow, the soft, spongy center of bones. When leukemia develops, the body produces large numbers of abnormal blood cells. The National Cancer Institute (NCI) says that in most types of leukemia, the abnormal cells are white blood cells. The leukemia cells usually look different from normal blood cells, and they do not function properly. Leukemia causes an accumulation of immature cells in the marrow and blood and as a result, the marrow can no longer produce enough normal red and white blood cells and platelets. According to NCI this results in such symptoms as:
  • anemia (Read about " Anemia ")
  • paleness and fatigue
  • easy bruising
  • easy bleeding (Read about " Bleeding Disorders ")
  • inability to fight infections
The American Cancer Society says leukemia can be categorized as chronic or acute. Acute leukemia progresses quickly. NCI says chronic leukemia progresses more slowly and permits greater numbers of more mature, functional cells to be made. Leukemia is also categorized by the cell type involved, either myelogenous or lymphocytic. Leukemia can arise in either of the two main types of white blood cells lymphoid cells or myeloid cells. When leukemia affects lymphoid cells, it is called lymphocytic leukemia. When myeloid cells are affected, the disease is called myeloid or myelogenous leukemia. NCI says the most common types of leukemia are:

It already is used for advanced CML and as a second treatment for CML patients who don't respond to interferon-alpha. Gleevec is now approved for the treatment of patients with all three stages of CML CML myeloid blast crisis, CML accelerated phase, and CML in chronic phase, either before or after use of other therapy. The only known cure for CML is still by a bone marrow transplant. Side effects: The most common side effects reported with use of Gleevec include nausea, vomiting, fluid retention, muscle cramps, fatigue, skin rash, and headache.

The U.S. Food and Drug Administration has announced that Gleevec (imatinib mesylate) has been approved for the "first-line treatment of patients with chronic myeloid leukemia (CML), an uncommon life-threatening form of cancer affecting about 40,000 people in the United States." First-line treatment means that Gleevec can be administered after CML has been diagnosed. The drug was originally approved in 2001 under the FDA's accelerated approval regulations.The federal agency believed more time was needed to determine whether Gleevec would continue to be as effective in arresting the leukemia as it had been when it was used after other drugs had been tried. "Today's (Dec. 20) approval represents continued efforts by government and industry to provide patients suffering from CML with additional therapies that have proven safe and effective through on-going research and clinical trials," said FDA Commissioner Dr. Mark B. McClellan. "With this new use, even more patients will have access to this product earlier on in their fight against cancer," he said in an FDA press release.

Secretary of Veterans Affairs THE VETERANS diagnosed with chronic lymphocytic leukemia, or CLL, would start receiving improved benefits, such as disability compensation and priority health care services, in about a year, Secretary Anthony Principi said Thursday.
Veterans Affairs expects to find about 500 new cases of CLL a year among Vietnam veterans, said spokesman Phil Budahn. About 2.6 million people served in Vietnam during the war and many of those who came home are still are alive.
