Low-Sodium DASH Diet Lowers Blood Pressure
The effect of reducing dietary salt on controlling hypertension, or high blood pressure, has been surprisingly controversial. But in a recent study, a collaboration of scientists, including researchers from Harvard Medical School and the National Heart, Lung and Blood Institute of the National Institutes of Health, suggest that reducing salt intake may dramatically lower blood pressure in people with or without hypertension, regardless of age, race, or gender.
The trial, known as the Low-Sodium DASH Diet study, tested the effects on blood pressure of lower levels of sodium intake combined with the Dietary Approaches to Stop Hypertension (DASH) diet, which is low in fat and rich in fruits, vegetables, whole grains, and low-fat dairy products. More than 400 subjects were assigned either a typical American diet or the DASH diet for 12 weeks. The salt content of every participant's diet was randomly changed every four weeks to one of three sodium levels: high (3,300 mg), intermediate (2,400 mg), or low (1,500 mg).

Pulmonary Hypertension Report: The Fen-Phen Epidemic In one of the most dramatic legal battles ever waged in the American Courts, consumer lawyers prosecuted the makers of the wildly popular diet drugs known as Fen-Phen (fenfluramine, phentermine, Redux and Pondimin) resulting in a $4 billion global settlement for victims of the harmful diet drugs. Consumer lawyers sought to prove that the weight loss drugs produced by the drug manufacturers were causing heart valve disease and a rare condition known as primary pulmonary hypertension (also known as PPH). The evidence about known risk of fatalities unearthed in the historic legal battle would shock six million consumers - the fen-phen combination of diet pills can cause serious heart valve disorders and primary pulmonary hypertension. In the case of the progressive disease of primary pulmonary hypertension, no known cure exists. As we enter 2003 - nearly six years after the diet drugs were pulled from the market - many Fen-Phen users are tragically learning they have or may later develop pulmonary hypertension or PPH. Symptoms of pulmonary hypertension include shortness of breath, fatigue, dizziness, fainting, discolored hands or lips, and swelling in feet and hands. The most common symptom of primary pulmonary hypertension is an increase in the pulmonic second heart sound, which is typically observed in a physical examination. In addition, the enlargement of the main pulmonary artery is common and is observed with a chest x-ray. One of the best methods for determining or diagnosing pulmonary hypertion is the echocardiogram. Unfortunately, primary pulmonary hypertension is often misdiagnosed or overlooked. The disease is rare and not commonly observed by many medical practitioners.

NEW BOOK SHOWS HOW TO LOWER BLOOD PRESSURE WITHOUT MEDICATION "What makes the DASH diet successful is its emphasis on healthy foods," said author and diet creator Thomas Moore, MD, assistant provost for research at BUMC, and professor and director of the Office of Clinical Research at BUMC. "Our research showed that no single food or nutrient is capable of decreasing blood pressure, but a diet rich in several vegetables, dairy foods, fruits and whole grains; and low in sugars and fat decreased blood pressure substantially in several individuals with hypertension." The first part of the book provides readers with information on how the diet works as well as advice on how to incorporate the diet plan into everyday life. The second half of the book provides 62 recipes created by a team of experienced nutritionists, practical informational resources and a form to help individuals track their new eating habits. Together with Moore, a team of researchers studied the affects of the diet on a group of 459 individuals with high and high-normal blood pressure. Results showed that the DASH diet reduced blood pressure in hypertensive individuals by an average of 11 points in systolic pressure and 5.5 points in diastolic pressure. "The DASH diet is a healthy diet for all individuals, but never change your medications without the advice and agreement of your physician," said Moore.

My research program is a combination of laboratory research on human lipoprotein metabolism conducted in the Department of Nutrition at the Harvard School of Public Health, and clinical trials in nutrition, cardiovascular disease and hypertension at the Channing Laboratory, Brigham and Women's Hospital. My laboratory research concerns the acute and long-term effects of diet on the kinetics of lipoproteins. These studies are determining the metabolic pathways for lipoproteins in plasma using a new system of lipoprotein classification and fractionation based on the presence of 2 apolipoproteins, apo E and apo C-III that affect metabolic rates. The changes in the metabolic pathways are being investigated in pre- and post-menopausal women taking or not taking estrogen, hypertriglyceridemia and normolipidemia, and diabetes and nondiabetics. The effects of dietary fat and carbohydrate are being studied in the fasting and postprandial states. Dr. Sacks's laborartory is conducting studies of lipoprotein subfractions in the Cholesterol and Recurrent Events trial to predict cardiovascular events, and differential response to treatment. I am the Chair of the Steering Committee for the DASH-2 trial (Dietary Approaches to Stop Hypertension). This multi-center NHLBI trial is studying the effect of dietary patterns and salt intake on blood pressure in persons with mild hypertension or high-normal blood pressure. High blood pressure may sometimes be secondary to diseases of kidneys or endocrine glands like the ovaries, suprarenals or pituitary; this may respond to treatment of the cause. Essential hypertension is the most common disease of the industrialized societies, particularly among the middle and old age groups. Changing diet substantially lowers blood pressure
The current average intake of sodium in the UK is 3.2 g, equivalent to about 8 g of salt, which can lead to raised blood pressure. The recommended daily amount of sodium (in the UK) is 1.6 g/ 70 mmol (equivalent to about 4 g of salt). In the US, the DASH (Dietary Approaches to Stop Hypertension) Study found that a diet that emphasises fruit, vegetables, low-fat dairy foods and wholegrain products and reduces the amount of red meat, fats and sweets lowers blood pressure, compared with a typical US diet. This trial studies the effect of reducing sodium intake in combination with the DASH diet on blood pressure in adults with and without hypertension.
Bottom line
The combination of lowering sodium intake with the DASH diet produces greater reductions in blood pressure than either the diet or reducing sodium alone. This combination is worth recommending for both the prevention and treatment of high blood pressure.

That includes the amount that is naturally in food as well as salt that has been added. If your blood pressure doesn't go down after two months, you probably aren't salt-sensitive and will need to find other ways to lower it.

Hypertension is an intermittent or persistent elevation of the blood pressure (systolic blood pressure above 140 mm Hg or diastolic blood pressure above 90 mm Hg) or (a systolic and diastolic pressure of 20 mm Hg above the normal baseline pressure). Hypertension has recently increased in incidence throughout the world. It is thought that the stresses of everyday life with a change in the dietary habits and lack of exercise has led to the increasing incidence of hypertension. Cause and Pathogenesis The cause of primary (essential) hypertension is not known. It is thought to be due to multiple risk factors. Known risk factors include a familial history of the disease, race, obesity, tobacco smoking, stress, and a high-fat or high-sodium diet in genetically susceptible individuals.

August 24, 2000 (Reviewed: December 13, 2002) Dieting and exercise are often the first treatments prescribed for people with mild or moderate high blood pressure (hypertension). A new study published in Archives of Internal Medicine provides further proof that that exercise, especially in combination with weight loss, can help keep high blood pressure under control. This study divided 133 sedentary, overweight men and women with mild hypertension into two groups. One group cycled, walked or jogged for 30 minutes, three to four times per week while following their usual diet. The second group followed the same exercise routine and also participated in an organized weight loss program. At the end of the six-month study, researchers found exercise alone significantly lowered blood pressure. Those that combined exercise and weight loss, though, showed an even greater reduction in blood pressure and lost an average of 7.8 kg (17 lbs) of excess weight. Among the latter group, 67% of those classified as mildly hypertensive at the start of the study had normal blood pressures after six months.

An article in the August 29 issue of Circulation adds to the body of evidence that demonstrates that a folate-rich diet that includes foods like dark green leafy vegetables can reduce blood levels of homocysteinea step that may benefit heart health. Homocysteine is an amino acid produced by the body as a by-product of metabolism. High blood levels of homocysteine have been linked to an increased risk of heart disease. Scientists think that certain nutrientsfolate, and vitamins B12 and B6may have a moderating effect on homocysteine, helping to lower harmfully high levels, thereby reducing the risk of heart disease. This latest analysis is an extension of an earlier project called the Dietary Approaches to Stop Hypertension (DASH) Study that looked at how different foods appear to affect blood pressure. Researchers divided study participants into three groups: one consumed a "typical" American diet. A second group consumed a diet high in fruits and vegetables. The third ate a "combination" diet high in fruits, vegetables, and low-fat dairy foods and low in total fat and saturated fat. Called a "combination diet" because it incorporated several kinds of foods thought to affect blood pressure, it is currently referred to as the DASH diet. The researchers tested the participants' blood levels of homocysteine, as well as folate, B12, and B6 at the start and end of the eight-week study.
