Ebola Virus Hemorrhagic Fever: General Information What are viral hemorrhagic fevers? Viral hemorrhagic fevers are a group of diseases caused by viruses from four distinct families of viruses: filoviruses, arenaviruses, flaviviruses, and bunyaviruses. The usual hosts for most of these viruses are rodents or arthropods (such as ticks and mosquitoes). In some cases, such as Ebola virus, the natural host for the virus is unknown. All forms of viral hemorrhagic fever begin with fever and muscle aches. Depending on the particular virus, the disease can progress until the patient becomes very ill with respiratory problems, severe bleeding, kidney problems, and shock. The severity of viral hemorrhagic fever can range from a relatively mild illness to death. What is Ebola virus?

(above) Ebola virus docks with cell membrane at middle left. Viral RNA (yellow) is released into the cytoplasm where it directs the production of new viral proteins and genetic material. New viral genomes are rapidly coated in protein to create cores. These viral cores stack up in the cell and migrate to the cell surface. Transmembrane proteins (purple) are produced which are ferried to the cell surface. Cores push their way through the cell membrane becoming enveloped in cell membrane and collecting their transmembrane proteins (spikes) as they do so. Examples of coiled virions are shown in the background.

Web posted at: 8:20 p.m. EDT ALICE, Texas (CNN) A leading Philippines monkey breeder said his animals probably were the cause of a recent Ebola virus scare in the United States. But he pleaded for officials to spare the monkeys' lives. Texas health officials Wednesday slaughtered 48 monkeys housed at the Texas Primate Center that may have been infected with the Ebola virus. The monkeys were part of a March shipment from the Philippines that Alex Lina, owner of Ferlite Scientific Research, said "probably" came from his center. "I'll be open to anything but don't kill the monkeys. They are my children. They are like babies to me," Lina told Reuters news service as he broke down in tears at his farm in Calamba, south of Manila. Lina's farm supplies American and European researchers. In 1989, all monkeys on his farm were killed after an outbreak of the virus, known as the Ebola Reston strain. Now he fears for the lives of the 1,600 monkeys currently at his farm.
Meanwhile, in the United States, scientists at an Army research center and elsewhere were testing plasma to determine if the virus at the Texas Primate Center had spread to monkeys housed in a separate building.

Das Ebolavirus ist ein RNA-Virus aus der Klasse der Filoviren. Sie werden als Filiae, wie der "Faden" bezeichnet, da sie unter dem Elektronenmikroskop wie sehr dünne Fäden aussehen. Es sind von diesem Virus drei Antigenvarianten bekannt: das Ebolavirus, das Marburgvirus und das Restonvirus, letzteres Virus scheint aber nur Affen erkranken zu lassen.

Ausblick, Prognose Da das Hauptproblem der Infektion mit dem Ebolavirus die ungeklärte Primärinfektionsquelle ist, werden viele Anstrengungen unternommen, diese zu finden. Während der letzten Epidemien wurden Exemplare vieler Vertreter der Tierpopulation der entsprechenden Gegend eingefangen und daraufhin untersucht, ob sie das Virus als Hauptwirt beherbergen. Ohne den direkten Nachweis eines Haupt- bzw. Primärwirtes bleiben Fragen wie: "Kann sich das Ebolavirus bei vielen verschiedenen Wirten einnisten, oder ist es möglich, dass er ohne speziellen Wirt im Dschungel lebt"? Es gibt aber Hinweise darauf, dass sich Menschen nach dem Genuss von Affenfleisch infiziert haben. Da diese Tiere aber selber auch an der Krankheit versterben, kommen sie als Primärwirt nicht in Frage.

Management of Patients With Suspected Viral Hemorrhagic Fever
INTRODUCTION The term viral hemorrhagic fever (VHF) refers to the illness associated with a number of geographically restricted viruses. This illness is characterized by fever and, in the most severe cases, shock and hemorrhage (1). Although a number of other febrile viral infections may produce hemorrhage, only the agents of Lassa, Marburg, Ebola, and Crimean-Congo hemorrhagic fevers are known to have caused significant outbreaks of disease with person-to-person transmission. Therefore, the following recommendations specifically address these four agents. The increasing volume of international travel, including visits to rural areas of the tropical world, provides opportunity for the importation of these infections into countries with no endemic VHF, such As the United States. Since most physicians have little or no experience with these viruses, uncertainty often arises when VHF is a diagnostic possibility. Lassa, Marburg, and Ebola viruses are restricted to sub-Saharan Africa, and the differential diagnosis of VHF will most often be made for illness in travelers to this region. Since 1976, no imported cases of VHF have been confirmed in the United States, but every year there are approximately five to 10 suspected cases. These guidelines review the clinical and epidemiologic features of these diseases; provide recommendations on diagnosis, investigation, and care of patients; and suggest measures to prevent secondary transmission. This document updates earlier recommendations, issued in 1983 (2), for the management of suspected and confirmed cases of VHF. Accumulated evidence shows that transmission of these viruses does not occur through casual contact; thus, some earlier recommendations for preventing secondary transmission have been relaxed. Similarly, therapy recommendations have taken into account recent knowledge of the effects of antiviral drugs.
