Not the best resource
While some of the pretest books out there are excellent for shelf exam preparation, this book is lacking in terms of how representative the quetsions are. The best resource for the internal medicine shelf in my opinion is the MKSAP For Students available in book and CD form from the ACP. The questions in there reflect the content and difficulty found on the actual exam. I scored well on the shelf using the MKSAP and supplementary reading on key topics like Chronic cough, asthma, CHF, COPD, DM, outpatient medicine, etc., etc., during the clerkship. ... Read more

this book was really similar to the information taught in our pathophysiology course so I found it very helpful. I never looked online for content because i thought there was plenty of useful information in the book.

mediocre book
I really *wanted* to like this book. I used NMS Surgery and NMS Pediatrics previously and thought both were excellent. This book might be helpful for others, who like me, have trouble learning by reading a list of bullet points such as in StepUp of First Aid. Some things in this book were explained really well. However, I found many typos, which made me nervous about the quality of the editing as well as the reliability of the information in the book. Some chapters have large chunks of text missing. For example, in the endocrinology chapter's discussion of oral antihyperglycemic agents, it completely leaves out sulfonylureas and biguanides. The additional online information is a nice idea in theory, but it isn't user friendly to assume someone will log on to a computer to look up a chart. If it's important, put it in the book. If not, leave it out. I hope this book will be updated someday and improved, since I think it has potential.

Covers some topics well, but severely lacking
I thought this book would be great, but it's really not!It lacks huge chunks of information about diseases.There are some disease processes that it just flat out doesn't even mention.It also has certain sections that require you to look online to read about.I bought the book to read on paper, not on my computer!It's sitting on my shelf awaiting to be resold or burnt.Overall it is a poor choice.

I can't really recommend anything else besides Harrison's or Cecil's if you're looking for complete coverage.But as any medical student would know, those are impossible to read.

BETTER OPTIONS OUT THERE!!!!!
I bought this book because I loved the NMS Q&A for family practice and figured if the question book is this good then imagine what the text will be like. The book is set up in outline fashion with sentence descriptions (not bullet points like Step-Up/First Aide) but many have little computer mouse icons next to them. This means go online and look at the text online. Problem is the text is not yet available online. I've e-mailed the publishers but I can't imagine that will do anything at all. Also the review questions at the end of each section are not very difficult (and definitely not the quality of Q&A family practice). Overall the book is good but it really needs the missing information (the mouse icons) to be complete. I would say us Step-Up when studying for the Shelf exam or boards and not NMS Medicine, atleast not until they get the online text working!!!! ... Read more

Recommended for oral board review
I would unequivocably recommend this book based on the number and strength of cases.

Well-organized and with 109 cases.(The cases are harder than those presented in a review course.)I appreciated the citing of "pearls" and critical actions.Interesting appendices, as well.

I did buy it via Kindle and was vaguely annoyed by picture quality and inability to quickly "flip" back and forth.

extremely helpful test preparation
Just took the oral boards and found this book extremely helpful in my preparation. Studying with this book works great when you're together with fellow EM trainees, but it's also fine with non-EM folks or even for reviewing by yourself. Each case has an area where the critical actions are well defined, but also a nice summary of emergency disease management little pearls about each topic. If you go through this book, you'll be extremely prepared and a lot more comfortable about the real exam.

Better than the other leading EM board review book
I studied with friends for the EM boards using both this book and another leading EM boards book, Emergency Medicine Oral Board Review: Pearls of Wisdom, and I felt that this book was far superior to the other, which advocates outdated information and has far fewer cases.(As an FYI, it seems that there are rumors that ABEM wants you to do non-evidence based things on the boards.Not true.)The other book supports that notion (ie -- you must hyperventilate all head trauma.Ugh.How 1992!).This one does not, and the authors actually reference the evidence for their conclusions at times.

This book has the most cases by far (> 100), but there are no triples.That is easily solved by, well, just having a friend give you three cases together.It is not a big deal at all.The cases are all classic EM, the kind of cases you should be practicing for the boards.No zebras, no off-the-wall scenarios, which is just like the real boards.

There is a small appendix of high yield information on peds, trauma, tox, procedures, and ACLS.Very useful.The appendix is smaller than in the other book, but it appeared to me to be more high-yield.

There are quite a few typos and a few factual medical errors, but they are mostly minor and/or obvious.Fairly typical for a first edition.

Emergency Physician
Drs. Okuda and Nelson have done an excellent job in compiling this oral board review. This text is a unique one, in that it addresses key issues in preparing for the Emergency Medicine Oral Board Exam. It helps to organize your thought process, by going through > 100 classic cases. It also allows you to practice for the exam by going through cases with a colleague.

Having just taken the ABEM oral boards, I highly recommend this book.

Excellent, comprehensive review book for ABEM Oral Boards

HAS NO ILLUSTRATIONS!
As of today there are 5 Kindle versions of this book for sale on Amazon. I bought 3 of them, and none of the 3 I bought included the illustrations. Still a great read, but the original version of this book has interesting drawings and photos. Too bad, I would have loved to have the complete book on my Kindle.

This version has no Pictures!
A.& C. of M. is a fantastic book indeed with it's "ancient" coverage of medical anomilies in one place!! It also had a brief cameo in the movie "The Breakfast Club."However this version of the book(Blue Unicorn Press) has none of the original pictures of which there wereover 100 in the original version (late 1800's) and in the reprints duringthe mid to late 20th century. Better than Swansons
I initially purchased Swansons to study with but quickly decided I hated it. There are soooo many typos and it doesn't provide very detailed explanations. Some friends were editing First Aid and I decided to give it a try. It's in the usual First Aid format, which I was used to. Provided good explanations and I don't think I encountered anything on the exam that I hadn't read in the book. My study plan including reading First Aid front to back and doing as many of the practice questions on the AAFP website (free) and old in-service exams as possible (ended up doing about 800 questions). Several of the questions were repeated word for word on the actual exam. Overall, First Aid was a great part of my studying and I will definitely buy it again when I'm up for recertification.

The only downside I found is that the Women's Health section is a little disorganized and could contain more material. That was my strongest area though so I felt prepared. If you haven't done ob or gyn in a long time though, you might want some additional materials. It also does not provide many sample EKGs for review.

Disappointed
I was disappointed after buying this review book for my Kindle and finding that you can't select the section you want to read from the table of contents and jump right to it.You have to scroll through page after page in order to get to the section you desire.Don't know if this can be fixed, but would be a huge improvement to the use of the book on the Kindle.

Great for quick review
This book is great for clinical rotations.It's reasonably portable.The sections are easy to navigate so you can find the snippet of information that you need.I like the memory tips too.

Too lengthy
It's kind of good for review if someone has time but I think it's as not as good as the other First Aids. I like the Mnemonics to help jog my memory.

Good summary but a bit pricey
Pretty good summary and add on resource for standard family medicine texts and question banks. The only downside is the price, which I think is a bit inflated because of the "first aid" name that everyone at the stage of requring board cert reviews is very comfortable with. ... Read more

studying for final and for nuc med boards
It has everything a Tech needs to know. It doesn't go too indepth, the pictures are pretty recent compared to other books, and good images of very particular things you need to know. Case studies in the back are great for quizing yourself, pearls give great highlights, and the tables in the back of great to know information with QC, NRC codes/regulations, etc are easy to understand and gets you the important info for taking your boards and almost any test you'll take in your Nuc Med Course.

Excellent Book
I am a Nuclear Medicine technology student and I highly recommend this book. It doesnt contain some of the most basic information but as long as you already have some basic knowledge of the nuclear physics and nuclear medicine itself, it is a great book. I has standarized protocols for various exams and gives which come real handy; also gives you the dosimetry for each exam which allows you to see which the critical organs for various exams are.

I highly recommend this book for anyone who is going into the field of nuclear medicine

A clear concise nuclear medicine text for the radiologist.
Concise. Easy to read. Excellent images. Three simple phrases for an excellent textbook. I was able to easily master and use this textbook on my first month of nuclear medicine and kept it "handy" for its tables and images. The pearls section at the end of each chapter were excellent for directing to truly key points. It is an excellent textbook for the _radiology_ resident and a good introductory textbook for the _nuclear medicine_ resident. Excellent reading with all the modern science information to support the whys and hows of traditonal eating. I especially like the chapter on creating a native diet that assists the reader to develop a balance in diet according to personal needs as those needs fluctuate. While this book has no recipes and no diets...if you're looking for a complete, sound nutritional guide to healing and health - buy this book.

Tradional Foods Really *Are* Your Best Medicine!

I am a Registered Dietitian that uses a holistic approach to nutrition and I feel this book is a valuable addition to any nutritionists library. Lacks a bit of detail in research areas, but is a good branch of the Weston Price tree.

good introduction to Traditional nutrition
Schmid's book was the first one I read on the work of Weston A. Price and Francis Pottenger. It is a very good overview of the subject of Tradtional diets. If you are wondering what "Traditional diets" are, they are simply the way people ate before big business took over food production, and made shelf life and profit margin more important than the nutritional quality of food.

Price was a dentist who embarked on a decade long research project in the late 1930s to find the healthiest people on Earth and study what they ate and how they ate it. His studies ranged all over the world, covering all different races. Schmid has done a good job of giving an overview of Price's findings.

The only issues I have with this book are that Schmid falls for the cholesterol scam in discussing heart disease, and that he also falls for the idea that the term "life expectancy" as used in statistics means the average age of death. (page 66) It doesn't. Life Expectancy as an arithmetic average would be reasonably close to the median age of death in a perfect Bell curve population sample, but such perfect samples only exist on paper, not in reality. The median age of death, that is the age by which half of the population died, was 57 in 1900. This means that half the population lived to be 57 or older. Kind of different than saying the average age of death was 45-50. In 2000, the median age of death was only 78, so there hasn't been as much gain as we are led to believe. Neither figure addresses the health or quality of life of people at those ages, either. A minor point in the grand scheme of this book. His discussion of life expectancy differences for those 40 and over on the rest of the page is still very much true and sets the record straight on the PR hype we are given about our current state of health.

For more in-depth information on fats and cholesterol, I recommend Mary Enig's book, Know Your Fats.

This is a great book, and will open your eyes to a better way to eat and improve your health. I heartily recommend it. ... Read more

Best Book out there for Women
As a medical professional myself, I must say that this has to be the most well written book,written by someone who knows, another woman. The book is full of easy to read information including sound advice that is backed up by Research, which is very important in my career. When I came across it I could not put it down, after 3 hours while still in the store I finally purchased it and could not wait to get home to continue soaking in all the wonderful information that Dr Hudson had to share. I will definitely be recommending this to my other colleagues and my personal friends. Actually a great idea. What better way to let that person know you REALLY care.

A godsent book!
I came across this book while trying to find alternative remedies for my sister's recurrent UTIs.
It is very well written with clear instructions regarding the dosage plan for each topic.
I'm glad to say that my sister didn't take antibiotics after all.
Loads of helpful info nicely presented. Even my mom is convinced to try the menopause plan for her! (And she has refused to do so since the beginning).
Thank you Mrs Hudson for an excellent book for women who want to give the naturopathic approach a try.

An unbelievably good read
I first came upon this author at the thrift store, and read The Last Report on the Miracles at Little No Horse book through without stopping, I loved it that much.I now have every one of Erdrich's other books, and have loved them all.The people here who rate this one or two stars have NOT read this carefully or thoughfully, or maybe shouldn't be reading anything more difficult than a Danielle Steele novel.These are so lyrical and full of heart and meaning, so deep.I am in heaven reading these, all of them!

My favorite fiction book...
Anybody who enjoys a book, needs to read this novel. It's the best you'll ever come across, believe me. From beginning to end, it's a book you won't be able to put down.

A multi-perspective and multi-generational tale.
Love Medicine tells a multigenerational story that spans many decades, lives, marriages, loves, and deaths. It is an ambitious novel that both attempts to provide a widescreen view of life as it interconnects across blood and generations while simultaneously reserving the right to zoom into quiet moments that, while they may seem insignificant at the time, blossom in import as author Louise Erdrich scales back her view to reveal the intricate nature of her story. The novel centers around the two poles of the Kapshaws and the Larmartines, two families who live on an Ojibwe reservation in North Dakota. These families are not made up of traditional nuclear units, and Erdrich must provide an intricate and looping family tree just so the reader understands who is related to whom.

Each chapter of Love Medicine presents itself as a short story, a common technique for a first novel. However, what separates Love Medicine from other novels who have taken the same approach is the way Erdrich utilizes the shifting point of view to provide a multifaceted view of characters and events. Most chapters are written from the first person and provide an opportunity for Erdrich to play with tone and voice that depends on the character. For example, Lipsha Morrissey, a teenager growing up in the eighties, utilizes videogames for metaphors. The death of a veteran returning from Vietnam is treated as an accident or a suicide depending on the author. The technique, if a bit less experimental even if simultaneously more grand, is similar to Faulkner's The Sound and the Fury.

By revisiting events, and even placing some events in non-chronological order, Erdrich's stories accumulate momentum and power as the novel progresses. As readers, we are aware that we are privy to only moments in a larger story that takes place off screen. In ways Love Medicine is like a collection of close photographs of a single skyscraper - a bird's nest on a ledge, an American flag, the sun reflecting off a window - without ever revealing the whole object. We recognize the whole from the aggregate because of our familiarity with both, and in the case of Love Medicine the whole is life from family.

Perhaps the single most impressive aspect of Love Medicine is Erdrich's prose. Her writing is just this side of magical realism, and while certain characters may believe in magic, Lipsha Morrissey believes he has a healing touch, because these very same characters are telling the story we are welcomed to doubt their powers. However, Erdrich's writing is often imbued with an effervescent mysticism. In the chapter "The Island" narrated by Lulu Nanapush, Lulu leaves her home to live in a cave on an island with Moses Pillager, perhaps a more surrealist chapter than the rest of the novel. Upon consummating her romance with Moses, Lulu, who would go on to father many children with many fathers, informs the reader: "I want to grind men's bones to drink in my night tea...I want to be their food, their harmful drink, to taste men like stilled jam at the back of my tongue." These moments of surrealism are equally matched by a prose that seems permeable and effervescent, as if the words can barely capture the events before us.

Erdrich is responsible for populating her novel with a myriad of characters whose lives bend and bounce off one another, and while we may not condone the actions of every one of them, there is a clear understanding that their actions rise from a shared pain. Because these characters are connected through a webwork of relations, their loneliness seems that much tragic.

Love Medicine Review
This was one of the most relatable books that I have ever read. It has lots of crazy family issues. Each chapter was told from a different person's perspective. At first I thought it was a bit strange, but I ended up enjoying it. The short stories of each person relate to each other and tell a bigger story. It's amazing how people involved in the same event can remember something so differently.

couldn't finish this book
After about 75 pages into this book, I was done. I've decided I'm not a fan of lyrical writing or short stories with no clear connection or flow between the chapters. Too disjoint for me, and some of the characters were too bizarre for me. This is my first attempt to read this author's work: I would be hesitant to try any of her other novels. This will go in the "donate" pile for my local used book store.

Not Impressed
This book isn't really that informative.I should have saved my money and just used my computer.

Mayo Clinic book on alternative medicine should be on everyones book shelf.It contains down to earth information about many simple procedures for good health.

Misleading title, inaccurate information
I purchased this book because I have been so happy with PDR for Herbal Medicines, 4th ed. (Physician's Desk Reference (Pdr) for Herbal Medicines). Which is another mainstream book on natural medicine. Unfortunately, this book is nowhere near that quality of research. It is a simplistic, inaccurate, and critically judgmental review of numerous approaches to natural health.

They use a stop light as a visual aide to show you their opinion of that particular item, a red light means not to use the treatment, or use it with a doctor's close supervision. The doctor's close supervision is a constantly reoccurring theme. A yellow light means use with caution, and a green light means it is safe for most people to use.

They give the herb goldenseal a red light. The Herbal PDR states the goldenseal is 90% effective against malignant brain tumors. It is more effective than any chemotherapy available.

Niacin they give a green light, stating that it can effectively reduce cholesterol, but then they go on to say it should only be taken under a doctor's supervision. A medical doctor recently told me that niacin was medicine's "dirty little secret" because everybody knows it is much more effective than any cholesterol lowering drug. But, there's no money to be made in it, so no doctor will tell you about it. Another natural alternative to a prescription for high cholesterol is vitamin C, it is discussed in great detail in Putting It All Together: The New Orthomolecular Nutrition.

It gives magnesium a yellow light and says that it is uncommon for most Americans to be deficient in it. We need 300 mg. of magnesium a day, and most people only get 250 mg. so there is a gradual depletion of our magnesium levels. It is one of the main causes of constipation, it is called for in heart cases, and it greatly reduces the chances of having a mentally retarded baby; the list goes on and on. If you have a health problem, a magnesium deficiency will seriously aggravate it.

When it does give interesting information, it doesn't give you the details to use it. For instance, it says that a study showed that vitamin C, E, beta carotene and zinc showed that it reduced the advancement of macular degeneration. But it doesn't tell you the dosage or even site the study so that you can look it up.

It gives homeopathy a yellow light, and says that there is no research to support its use. There is an immense amount of research supporting the efficacy of homeopathy. One researcher said "Either double blind studies don't work, or homeopathy does." He obviously went into the study believing that homeopathy didn't work. Hence the need for double blind studies, so that the researcher can't influence the outcome.

It says that generic supplements are as effective as brand names, which is simply not true. Quality varies greatly by manufacturer in all things, including herbs and supplements.

If you are educated on the subject of natural health, this book will make you furious because it is so inaccurate and misleading. If you are just getting interested in natural health, I recommend you start with Chemistry of Man (Man Series, Second Edition) instead.

Bible Of EM

This book is a great read compared to any other text out there for EM. It is long but the time invested in learning and reading it worth-while.

A masterpiece in Emergency Medicine
Here as before, the authors have brought Emergency Medicine to a new height.This book, like the last two editions, revamps the subject in a form that is readable, and to the point without the superfluous "fluff" seen in other books.

It comes in two volumes (instead of the three like prior editions).On-line access is a definite plus when I am at work and can't haul around a big fat textbook.I can search for things on my computer.

I totally recommend it.It is one of my 'must haves' in my armamentarium of EM.

No complaints. I purchased the book not the CD.Pleased with online access, as internet is available everywhere. This is far better than Harwood and Nuss for completeness of content. So many of the chapters there have leave information out.

No CD, just a "Pin Card"
I spent $300 for what appears to be a CD case, within the box is a card with a scratch-off area for a password to get online and sign in, NO CD! On the Amazon site if you look under "Product Details" it clearly states that this is a CD, I assure you that it's not (as of 3/30/06).If you're like me, you go to multiple ED's and carry a laptop, or you may want to read outdoors or on an airplane.Unless you're always able to connect to the internet this product is useless.In addition the back of the box is states "May Not Return if Opened".We'll see about that, given that this is false advertising.The old 5th edition works fine on CD rom, why did they have to go and screw it up? ... Incomplete
Nothing on ME (CFIDS). ME is a common and extremely disabling NeuroEndocrine Immune Disease associated with two retroviruses, rare b-cell lymphomas, etc.

Pros - really a pocket book, fits your coat and isn't heavy
Covers the main topics of Surgery, Emergencies, different Internal Medicine specialties (Neuro, Cardio, Rheuma, Infecto, . . . )in a great way
The first part is really awesome: not only for its bed-side manners, but for their words in critical thinking. It goes on and on: the authors remind you every other page that you're treating patients -flesh & blood people- not illnesses. And they don't do it in a "oh-boy-lecture" fashion, but with phrases and mind exercises

Cons- it was impossible to cover every disease here. But you`ll find some uncommon conditions in the eponymous section (which can prove useful while in the rounds or for the on-call-quizz-attending). And please keep in mind: it is NOT enough for learning Medicine. You still need to work hard and study. This a "reminder" for themes you've already read or an "introductory review" for the themes you'll eventually check afterwards.


Tip: buy it w/ OHCS Oxford Handbook of Clinical Specialties: Book and PDA Pack (Oxford Handbooks Series) and you're done! 'cause it includes the other main specialties as ENT, Pediatrics, OB/GYN, Orthopedics...

A great book to read...
But unless you are a Doctor or want to go to prison, don't practice most of the proceedures in this book.
I loved it anyway even though I am a humble medic.

"Ditch Medicine" is very informative and is written in a concise manner.
No single publication contains all the information that we would like but this book should be in your personal library if you have an interest in emergency field medicine in an austere environment. I highly recommend this book.I also highly recommend emergency medical training prior to employing many of the procedures outlined, if at all possible.

Advanced procedures indeed!
Not just how to treat an amputation, but surgical procedures for performing an amputation!Other topics include IV cutdowns, nerve blocks, chest tubes, and suturing.While the procedures are numerous, I can't imagine learning how to perform them successfully from the text alone.The author assumes his reader is well-equipped with both gear and a basic knowledge of medicine.If you have the proper gear and can understand the instruction, you probably didn't need the book.Further, if you have no experience with IVs or suturing, performing a cutdown seems ambitious.As stated by another reviewer, much of the information feels dated.Even moreso, extreme amounts of information are left out.Nevertheless, while the topics in this book are covered better elsewhere, it is still interesting.Useful?Probably not.I can't say the instruction is terribly practical.Neither do I find it helpful to reference.But it definitely has cool factor and I'm glad I have it!

Good background and tech
Basic bring'em back alive tech, deals with using what you have on hand as well as what a funded medic has in their kit! I liked the historical notes as well!

Solid no nonsense basics
For the cost this is a good reference.It is written in plain english and is easy to understand.It is of particular use to the health care professional that has never operated or has limited experience in austere environments.It has some deficiencies (pictures/drawings are not the greatest, some of the material is dated, no real discussion of permissive hypotension etc...) but these are forgivable in a volume of this size, cost and ease of reading.

Every Licensed massage therapist should have this book. Why? Great information -
These books have a series of articles about various medical topics. While I found it very informative, it is written for med students but it is not difficult to understand.

Must have for Reference
This is a great book.I've used Harrison's as a required book for my school, but it is not as organized and thorough as Cecil Medicine.Cecil is gives you a clear-cut thorough anatomy, and pathophysiology, along with up-to-date treatment and interventions for each disease discussed. The set up is not like a boring medical textbook.Reading this book sparks my interest to know more about the disease I am learning.It is a heavy book, but worth it! Even the MD that was precepting me for Internal Med. highly recommended this book.

Hi.... i really recommend this book for all the medschool students, it explains each disease in a very easy way to understand and learn about it, also the way it is structered helps to have a logical sequence of the disease (intro, epidemiology, physiopatology, etc).

Having just finished this book, I'm better informed and and more depressed.
I'm an oncologist who likes to think that oncology studies are better controlled and more fastidious than other studies.But as I read this book, I wondered whether I was deluding myself about that. More and more of our studies are pushed by pharm. companies and fewer bynationally run cooperative groups.Also, I was disconcerted and somewhat nauseated to think I may be writing prescriptions that don't do what they're advertised to do.I've always had a healthy skeptism for drug reps - or at least I thought I did - and put up with them so that my staff could get lunch.No more.I will not give them any more of my time.

This is a book that all doctors should read, which won't happen of course.It's an eye opener, quite dismaying.I'm grateful to the author for writing it. We (I) need books like this to maintain a critical eye.

when is an ethicist ethical?
There was some careful product placement here and misleading advertising. Carl Elliott's articles in the New Yorker describe lurid medical misconduct. The doctors in this book are only evil enough to allow themselves to be swayed in their prescribing by gifts of pens and coffee mugs.
It's not a textbook of medical ethics. There's nothing about Kant's categorical imperatives or the Golden Rule or dropping bombs on Nazi German cities. In the opening chapter he describes an interesting and classical quandary. Two drugs are known to be effective for a disease. It would help us to know which is best. The victims of the disease are mentally ill, or are children in Africa.Solving a problem like this demands a lot of careful logic and fancy statistics (calculations of power requirements, number-needed to-treat, optional stopping etc). Elliott gives us none of this but gives a highly colored anecdotal account of the people involved (the American ones anyway). He writes as a journalist rather than as a philosopher and flogs some rather dead horses in order to produce readable anecdotes
His accounts of doctors being influenced by free meals and rides in fancy limousines rather bemused me. Most of those I know can afford their own food and cars, and make more money from a day in the office or O.R. than from giving an out-of-town lecture.
It's partly about medical ethicists, who apparently live in their own circles and get free meals and rides in fancy limousines and are corrupted by pens and coffee mugs. Is seems the only way for an ethicist to stay incorruptible is to have a private income held in a blind trust.
That all sounds rather negative, but this is an intersting and well-written book that I enjoyed reading and learned a lot from.

great book-must read for every patient
I have mnay family menbers who were doctors or are doctors now and have worked in chemical/pharma industry. This book shows how money and the pharma indusrty is no better than and maybe even worse than others than promoting their new products .
Due to long product cyles and billions of reserach dollars,drug companies must
devolp what I call the "home run" drugs , i.e. drugs will sell billions of dollars and not the drugs needed by a few million - it just deos not pay just sell few million, the drug must sell billions. To accomplish this, doctors must be convinced or bribed by free travel , classes or just put on the payroll.

Superb portrait of Modern Medicine
As a person who has written about the pharmaceutical industry for years, been a pharmaceutical executive and both a critic and supporter of the industry, I highly recommend this book for its unique perspective on the practice of medicine, the drug business, and the many people (both good and bad) involved.Dr. Carl Elliott has done an excellent job of using his own background in medicine (his father was a physician), his research in philosophy and his own perspective as an MD and professor of bio-ethics, to give a very balanced overview of the many problems besieging the modern pharmaceutical industry. Please don't let all these credentials make you believe this is yet another pedantic study of medicine.The writing is superb and compelling, the stories both often funny, sad and touching and the narrative clear and straight forward.Best of all, while the book maintains a high level of scholarship, it never loses its human touch.As an example of why anyone who has taken a drug should read this book, is Dr. Elliott's examination of the types of facilities and the kinds of people on whom drugs are frequently tested.(It is shocking, but true that drugs released to millions of people are often tested on a limited number of people who are not at all representative of the population as a whole.)Dr. Elliott also does a superb job of helping the reader navigate through what has become a vast, medical-industrial complex: including government regulators, universities, physician groups, medical journals and "continuing medical education" that is sponsored by drug companies.The reader will ask after reading this book just what the difference between marketing and science really is?As someone who has worked in the industry, I found it refreshing that this book does not take "cheap shots," but portrays the many ambiguities involved in medical decision making.(Elliott gives several hilarious but true portraits of drug reps who have an uncanny ability to make themselves likable even when they are peddling snake oil.) There is also a tragic portrait of a mother whose son dies in a botched drug trial, for which no one takes responsibility. The book paints full portraits of the "high and mighty" involved in medicine and makes human even some people who have committed some of the worst conceivable breaches of ethics.In sum, well written, easily understood, amusing and appalling -- and very finely researched.

Biased, Not as Good as Existing Books -
Author Elliott reports that the culture of deception within the medical community has become pervasive - driven by profit-maximizing drug companies. He buttresses that conclusion with innumerable vignettes of how they have withheld negative outcomes information about their products, while creating slanted supportive reports in ghost-written medical journal articles, CME presentations, and various forums for both physicians and the public. Supportive physicians are likely to find themselves basking in the sun on a drug-company vacation. Meanwhile, the ranks of drug reps have ballooned to one for every 2.5 physicians - handing out lots of free lunches and small gifts, promoting high-volume 'off-label' uses in sly ways that don't create FDA punishment (the temptation to avoid replicating development costs is enormous), and analyzing 'script-tracking' data from pharmacies to identify the most profitable physician 'high-writers.' Little of this is news, and most all of it is better told in Marcia Angell's "The Truth About Drug Companies," and John Abramson's "Overdosed America."

The one 'surprise' in "White Coat, Black Hat" was learning how much is charged for scientific journals as a result of their having a stranglehold on university libraries. Annual subscriptions can run $20-30,000; these high rates restrict access to important research, most of which was funded by public money, contributed free by authors, and also peer-reviewed without charge as well.

The author also bemoans the loss of much clinical trial 'business' from academic medical centers to for-profit entities. However, I can easily understand why - as an underling at an academic medical center I was amazed by the lead physician's belief that the center's economic existence was justified by the key role it played in research, while doing nothing to make the activity less costly or faster. While reading "White Coat, Black Hat" I sometimes also got the feeling that Elliott's attitude was similar.

Now An estimated $15 billion in government funding from China will be available over the next 5 years for drug development (Biotech in China, 2/2010). Examples of why firms are moving to China include the following:

Dr. Reddy's Laboratories (India) can conduct preclinical trials for 40-60% less than in the U.S., while Aurigene (India) can conduct chemistry work for a quarter of the cost. Eli Lilly contractor with ShanghaiBIo to collect 100 samples of specific human tissue samples in three weeks after it was unable to do so in the U.S. Tissue collection can cost as much as $2,000/sample in the U.S., vs. $300 in China. Chinese employees with PhDs from top universities can be hired for about $15,000/year. Regulatory approval in Korea for a clinical trial can be obtained within 30 working days. An estimated 87% of trials in the U.S. are behind in recruitment and enrollment, taking 3-6 months from approval to enrollment of the first patient.

Massive economic change will occur - as of May 2008, NIH identified more than 56,000 clinical trials worldwide, including 32,410 in the U.S., but only 750 in China and 670 in India. Preclinical testing monitors the effects of the drug when assimilated into a biological system, and is performed on animals and through laboratory tests. Phase 0 Clinical Trials are an optional first round of test on human subjects and involve delivery of very small drug quantities - designed to eliminate poorly performing drugs early. Phase I Trials are generally conducted on a small body of healthy volunteers to test a drug candidates safety. Phase II Trials assess drug functionality and are conducted on larger groups to determine dosage. Phase II are the most costly and time-intensive. They involve groups of hundreds or thousands of volunteers, and usually compare results with standard treatments. Phase IV trials track for long-term safety effects after a drug has been approved for commercial sale.

Clearance of an experimental drug in the U.S. normally takes 3-6 months. In many cases the entire process for new cancer drugs takes about 12 years and costs around $1.3 billion. (On the other hand, one must not forget disasters like Fen Phen, Vioxx, and Thalidomide.)

Bottom-Line: Drug company dishonesty is a serious problem; so are costs and bureaucratic delays for new drug development. Again, however, I recommend Angell's and Abramson's works instead.

  Just my two sense-Great book for any doctor to have on the shelf
  This is an amazing book to keep around.It has an extraordinary amount of information in a listed outline format.The conditions are listed by complaint ("Low Back Pain", "Cough").This is extremely helpful for any medical student or resident trying to work on tuning their differential diagnosis skills or for looking up how you would work up a condition.It gives all the latest guidelines on how to treat and diagnose conditions along with the presentations that are most likely seen in a clinical setting.It is too thick of a book to use on clinical rotations but would be an excellent primary source for a resident in family med or even internal medicine.I used it mostly as a reference while in family medicine clerkship and was happy enough with it that I bought a copy.Frankly, it is the bread and butter information that any respectable doctor should know.For me, it covered 90% of all complaints that I saw while on my clerkship.The only real down side is that the book is rather advanced.If you haven't been to 2-3 yrs of medical school and you don't understand that pathology of a disease just by reading the name of the syndrome (ie- Reiter's syndrome) this book is too complex for you.

  Good Reference
  I used this for family medicine clerkship.It is good because it outlines all major categories of ambulatory illnesses/problems and explains current treatments/recommendations.Some of the information goes beyond what you are looking for when preparing for the shelf (i.e., advice on patient counseling) but, at the same time I emphasize the fact that this book does a great job of concisely summing up the current guidelines and classifications for all sorts of diseases, and uses tables effectively.I recommend this book as a good reference, but you will definitely want to use a source that has a bunch of questions, like Lange's family medicine clerkship question and answer to supplement this (I took the shelf today and actually saw one question on the test that was identical to a question in the Lange question and answer book).All in all, a good buy --- especially since it is a fairly small book, easy to keep with you to study and concise and informative.

Magical
I haven't felt the same about the plants and trees around me since the day I picked up Plant Spirit Medicine at the suggestion of a friend.This book is beautifully written and full of fascinating stories and ideas.It weaves a subtle magic just by the reading of it.If you want to open your heart to wonder and the possibility that healing can arrive in forms you've never even dreamed of, then definitely buy this book.And you might as well order two copies straight off because it's the kind of book that you can't wait to share with your friends, but you won't want to let your copy go. The best book on the subject of plant spirit medicine
This book is a deeply reflective survey on plant spirit medicine.It is pretty obvious the author has extensive experience on this subject traveling the world and working with some of the visionary keepers of this knowledge now being lost to our contemporary world. It is also pretty clear that one goal of the book is to document this knowledge for non-western peoples and preserve it for prosperity on behalf of the wisdom keepers of this ancient medicine. As the superstructure continues its material collapse, those left standing will need this type of wisdom to carry forward the human enterprise.

The discussions on the way indigenous healers approach this subject as opposed to the way Westerners or Eurocentric people approach it will invoke disorientation for those nursing off of western rationalism. The indigenous world view turns upside down modern rational thought by presenting a vision of the world any westerner would dare not go, let alone contemplate since it is so far outside his domesticated and routine life.

This book is by far the best book on a complicated subject but more importantly, it provided some tools for working with the spirit world that are powerful. You won't find this kind of authenticity in most narratives on the subject. If personal transformation and non-western thinking is high on your wisdom meter, this book delivers the goods in countless ways.

Cowen is a visionary of the highest order. Read this book again and again. It might transform your life.

Relationships are about you and the plants
I am highly impressed by Cowan's approach to this topic.Instead of giving you impersonal information about plants and animals, his approach is to encourage the reader to develop direct connections with the plants.This is not easily done for several reasons, but Cowan makes it more accessible to people.

I think the most important part of his work is how he helps the reader understand that having a relationship with a plant is more than picking it, eating it, and waiting for it to fix you.

If you need someone to tell you how you relate to plants or animals, look up someone encyclopedic like Ted Andrews.Cowan's approach is much older and more personal.He won't do the thinking for you, but becuase of that you will actually have the opportunity to develop a connection with plants instead of an object/user relationship.

Plant Spirit Medicine
I read, and was deeply touched by the author's intelligent, knowledgeable and truthful writing. Elliot Cowan is one of those treasures you'd love to spend an afternoon with, or be a life-long friend. People should read this book.

Marking the end of arrogance

One of the classics
This two volume series is what most fellows(trainees) studying Neonatology use. I would not recommend it for the average medical student or even resident unless one is seriously considering Neonatology as a career. It packs a lot of material in two volumes, is well edited, and easy to read but most residents will not have the time to read it during their rotations in Neonatology.

I do not think most lay people will get much out of this although one reviewer found it quite useful. The book assumes a knowledge of basic physiology and medicine that would make it difficult to fathom for many people.

I would recommend "Care of the High Risk Neonate" by Klaus and Fanaroff as a better book for medical students wanting a grasp of the subject or the "Lange" publication on Neonatology.

With these warning I would give my heart endorsement for this book. I used it twice for passing the boards and then renewing my boards in Neonatology and found the most useful source of all. I used it with a DVD from the "Pediatrix" organization which references this source extensively.

What an experienced Neonatal Nurse Practitioner has to say...
This is the book you must have if going into neonatology or neonatal nursing.This is the gold standard and the most up to date information in the field.I have had the privilege of working with the authors and many of the contributors for years now,and I continue torespect and admire their work and knowledge.It's heavy and worth every ounce!

Very accessible
I'm an average everyday bean counter with no background or interest in medicine.I just bought this book because my kid decided to come out at 28 weeks, and let's be honest:Doctor's are too afraid of lawsuits to tell the rest of us lay people all the goods.However this book is great and very accessible for the average high school graduate, such as myself, to understand what's going on.Granted, wikipedia is also nice because doctor's like to use words like "emesis" when "vomiting" will do, however the concepts are presented very clearly.I recommend it to any unfortunate soul who's kid is in the NICU and wants to know what's going on.Of course, if we can understand it, I'm not sure how helpful this book is to real doctors who do this for a living.

Excellent Book
It is difficult to read, no doubt about that.However, it has a edge of giving the reader the required concept.Also it is unanimously known that the neonatology board questions contain a lot of questions, foundation of which are laid on the themes , well described in this book.
Definitely recommneded for any body really serious in neonatology.The latest chapter of bioethics and legal medicine is worth mentioning , as it has not been covered in any other neonatology text book. ... Read more
